Nurse's Office

A physical for all juniors is required. Physicals should include confirmation of the student receiving the Meningococcal Vaccination (MCV) after the age of 16.

Physicals for juniors can be turned in during their sophomore year.

Please email the form (marie.kozak@cohs.com) or have your student deliver it to the Nurse's Office. A copy of the PIAA physical with immunizations is an acceptable form of a physical for all juniors.

Health services are provided by a Certified School Nurse of the Marple Newtown School District. The school nurse adheres to the MNSD polices. The health service carries out state mandates and promotes healthy lifestyles.
