PIAA/Family ID

PIAA Paperwork

In order to participate in athletics, all physicals need to be uploaded to Family ID.

Click here to access the PIAA paperwork required for physicals.

All physicals need to be signed and dated by a physician after May 1, 2024 for the 2024-2025 academic year.

Family ID Registration

All physicals must be uploaded to Family ID.

Family ID submissions need to be completed by the first day of practice. Athletes are NOT permitted to participate without an up-to-date physical.

Athletes who participate in multiple sports throughout a year MUST resubmit a Family ID submission every season.

- Under "Programs", select "O"Hara Spring Athletics"

- Under "Sections", select your desired spring sport

- Upload an up-to-date physical and fill out all necessary information

- Click "Register Now"
